UnitedHealth Group’s Pleasant Valley Home Health seeks a Registered Nurse to provide and direct nursing care in patients’ homes, coordinating with physicians and the care team to ensure continuity and quality of care. You’ll supervise aides, perform supervisory visits, and ensure documentation meets policy and payer requirements.

The role requires an active RN license, current CPR, and strong communication and organizational skills.

Qualificações

• Current RN licensure in state of practice.
• Current CPR certification or ability to obtain within 90 days.

Responsabilidades

• Develops and coordinates the plan of care with physicians and health care professionals.
• Supervises home health aides and licensed practical nurses; provides instruction and assigns tasks.
• Performs supervisory visits and ensures documentation and care delivery meet standards.
• Completes assessments (OASIS), medication reconciliation, and aligns care with payer requirements.
• Communicates with physicians, patients, families, and agency staff to ensure quality care.
• Participates in on-call and weekend rotations as needed.
