Job summary

UnitedHealth Group is seeking a Registered Nurse to provide and direct nursing care to patients in their homes, coordinating the plan of care with physicians and the care team. This role includes assessments, ensuring eligibility, and revising care plans with other providers to maintain continuity of care.

The ideal candidate will hold an unrestricted RN license in the state of practice, have strong communication skills, and be able to work independently in a field-based environment.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification or ability to obtain within 90 days of hire.
• Ability to work independently and manage time effectively.
• Strong written and verbal communication skills.

Responsibilities

• Provide high-quality clinical nursing services within scope of practice and infection control standards.
• Coordinate care with the patient/client's care team from admission to discharge.
• Complete clinical nursing assessments per program requirements and payer needs.
• Ensure patient eligibility and medical necessity as defined by payer and agency policy.
• Develop and revise individualized plans of care with other providers.
• Ensure plan of care frequency/duration meets patient needs and obtain physician approval.
