Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to reliable transportation.
• Preferred CPR certification and ability to complete within 90 days.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with physician and health care professionals.
• Supervises home health aides and licensed nurses; provides instruction and assigns tasks per state and federal regulations.
• Performs required supervisory visits and ensures care aligns with policy and regulations.
• Completes assessments (OASIS) and medication reconciliation accurately and timely.
• Documents visits and coordinates with physician, patient, family, and care team.
• Contributes to quality improvement and ensures continuity of patient care.
