Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and coordinate nursing care in patients' homes, in alignment with physician orders and agency policies. The role emphasizes collaboration with physicians, aides, and the broader care team to maintain continuity of patient care across settings.

Primary responsibilities include planning and reviewing care, supervising staff, and completing OASIS assessments, with an emphasis on patient safety and quality outcomes.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License or access to dependable vehicle/public transportation.
• CPR certification preferred or within 90 days of hire.
• 1+ years of Home Health experience (preferred).
• Ability to work independently and strong communication skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises home health aides and licensed nurses, provides instruction, and assigns tasks according to regulations.
• Provides required supervisory visits and ensures plan of care compliance.
• Completes comprehensive assessments (OASIS) and medication reconciliation.
• Documents patient visits per policy and payer requirements and coordinates with care team.
• Communicates with physicians, patients, and families to ensure quality care and service.
