Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team serving Houston and Klein, TX. You will evaluate patients in their homes, develop individualized plans, and deliver evidenced-based interventions to improve mobility and reduce pain.

Qualifications

• Active TX Physical Therapist license.
• Professional liability insurance coverage.
• Graduate degree in Physical Therapy.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to different patients.
• Up-to-date knowledge of evidence-based treatment techniques.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in home health.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor patient progress and adjust plans to achieve outcomes.
• Accurately document evaluations, treatment sessions, and progress notes.
• Collaborate with physicians, nurses, and other therapists.
• Conduct home safety assessments and recommend fall prevention strategies.
• Deliver compassionate, patient-centered care to empower independence.
