Job summary

PARS Therapy is seeking a compassionate Physical Therapist (PT) to join our home health team serving Houston and Klein, TX. The PT will evaluate patients in their homes, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility and reduce pain.

Ideal candidates hold a Texas PT license, graduate degree in PT, and have experience in home health or rehabilitative settings.

Qualifications

• Active Physical Therapist license in Texas (TX)
• Professional liability insurance coverage for TX Physical Therapy license
• Graduate degree or higher in Physical Therapy from an accredited program
• Experience in home health or rehabilitative settings preferred
• Up-to-date knowledge of evidence-based treatment techniques and best practices
• Excellent communication skills and a friendly, professional demeanor

Responsibilities

• Evaluate patients’ physical abilities in a home health setting
• Develop and implement individualized treatment plans to restore function and mobility
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs and safety techniques
• Monitor progress and adjust plans to achieve outcomes
• Document evaluations, treatment sessions, and progress notes in digital platforms
• Collaborate with physicians, nurses, and other therapists
• Conduct home safety assessments and reduce fall risk
• Deliver compassionate care to promote independence

Essential Job Functions:

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies
• Monitor patient progress and adjust plans as necessary to achieve optimal outcomes
• Accurately document evaluations, treatment sessions, and progress notes using digital platforms
• Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists
• Conduct home safety assessments and make recommendations to prevent falls or re-injury
• Deliver compassionate care while empowering patients to achieve greater independence

Requirements:
