Job summary

The position requires active licensure, a valid driver's license, ability to work weekends, and CPR certification or timeline to obtain it. Compensation includes per-visit and hourly pay with a comprehensive benefits package.

Qualifications

• Current license to practice physical therapy in the state of practice.
• Current driver’s license with access to a vehicle or reliable transportation.
• Availability to work a weekend rotation.
• CPR certification or ability to obtain within 90 days of hire.
• Ability to perform physical tasks including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state-specific laws for home care PT, work under supervising PT’s plan of care.
• Instruct and assist patients with active and passive exercises, gait, ADLs, and mobility.
• Observe and report patient responses to treatment to the supervising PT and care team.
• Educate patient, family, and care team about therapy within the scope of practice.
• Function in diverse home settings with respect to all patients.
