Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patient needs and functional status to develop therapy plans.
• Provides therapy within scope of practice and in coordination with health care team.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and caregivers about plan of care and progress toward goals.
• Documents visits and submits progress notes per policy.
• Develops PT plan of care within state guidelines.
• Reports outcomes of evaluation and frequency of care.
• Adapts to home environments and diverse patient needs.
