Job summary

UnitedHealth Group’s Ms HomeCare unit seeks a licensed Physical Therapist to deliver home health services. You will assess functional status, plan care, and provide therapeutic interventions to improve mobility and comfort, coordinating with physicians and the care team.

Qualified candidates hold current PT licensure in the state, a valid driver's license or access to reliable transportation, and CPR certification is preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle, or public transportation.
• CPR certification preferred (within 90 days of hire).

Responsibilities

• Evaluates patient functional status and PT needs.
• Assists in developing therapy plan of care.
• Works within scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment.
• Instructs patient and family on plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes.
