Job summary

Perham Health in Perham, MN is seeking an Occupational Therapist to deliver skilled home health services, evaluate functional limitations, develop treatment plans, and support independence in the patient’s home.

This casual/PRN position requires Minnesota OT licensure eligibility, NBCOT if applicable, and at least 1 year of clinical experience; a valid driver’s license and strong documentation skills are essential. You will work with an interdisciplinary team to maximize patient outcomes.

Qualifications

• Graduate of an OT program accredited by ACOTE or successor.
• Current licensure or eligibility for Minnesota OT licensure.
• NBCOT certification where applicable per licensure requirements.
• AOTA registration preferred.
• Minimum 1 year of clinical experience; home health experience beneficial.

Responsibilities

• Evaluate patients to determine functional limitations and rehab needs.
• Develop and update individualized OT treatment plans.
• Provide home health OT services to improve independence and safety.
• Document progress and modify plans as needed in EMR.
• Coordinate with RN Case Manager and physicians for patient care.
• Educate patients, families, and caregivers on rehab techniques and home programs.
• Recommend and order adaptive equipment as appropriate.
• Participate in department meetings and ongoing professional development.
