Qualifications

Location: Ohio | Home Health | Full-Time/Part-Time Opportunities

Compensation Details:

• $45 per point. Mileage is paid at $0.43 per mile. Milage is not paid on travel for the first or last visit of your workday.

• OASIS Discharge is 1.56 points per visit

• OASIS Non-Billable Discharge is 0.4 points per assessment

• OT Discharge 1.5 points per visit

• OT Resumption 1.5 points per visit

• OT SOC 1.5 points per visit

• OT treatment visits 1 point per visit

• OT Recertification 1.5 points per visit

• OASIS Recertification 1.56 points per visit

• OASIS Resumption 2 points per visit

• OASIS start of care 2.5 points per visit

What You'll Be Doing

As our Occupational Therapist, you'll bring therapy directly to patients where they live—their homes and communities.

You’ll:

• Evaluate patients and create personalized treatment plans

• Set meaningful short- and long-term therapy goals

• Provide therapy in real-life home environments

• Work with patients, families, caregivers, and the care team

• Recommend or create adaptive equipment when needed

• Track progress and adjust treatment plans as patients improve

• Complete accurate and timely documentation

• Educate patients, families, caregivers, and staff

• Supervise and support Occupational Therapy Assistants (OTAs)

• Help patients become more independent and confident in their everyday lives

What We're Looking For

We're looking for an OT who is:

• Licensed as an Occupational Therapist in Ohio

• A graduate of an accredited OT program (MS/OTD or qualifying BS/equivalent experience)

• Current with required certifications, including CPR

• A strong communicator and team player

• Comfortable working independently

• Organized and dependable

• Compassionate, patient, and adaptable

• Comfortable working with patients with physical, emotional, or cognitive challenges

• Willing to travel for home visits

• Comfortable using computers for documentation and communication
