Responsibilities
- Perform comprehensive Physical Therapy evaluations using standardized tests.
- Develop and implement treatment plans per physician orders.
- Create home exercise programs with caregivers.
- Collaborate with other providers to coordinate care.
- Meet weekly visit targets as required by the agency.
- Maintain timely chart updates, progress summaries, and call logs.
- Adhere to all agency policies, rules, and applicable laws.
