Job summary

Qualifications
- Active Texas PT license required.
- Innovative mindset with reliability.
- Flexibility to adjust resources per business needs.
- Proficient with common workplace tech (laptop, phone, email, browser).
Responsibilities
- Perform comprehensive PT evaluations using standardized tests.
- Develop and implement treatment plans per physician orders.
- Create home exercise programs with caregivers.
- Collaborate with other providers to coordinate care.
- Complete required patient visits weekly.
- Meet deadlines for chart updates and progress notes.
- Engage with agency personnel to align on expectations and standards.
- Comply with agency policies and applicable laws for home-health PT.
