Job summary

Responsibilities

• Assist with ISP information gathering, planning meetings, and service reviews.
• Implement ISP goals, BSPs, and Nursing Care Plans.
• Assist with activities of daily living and medication support as authorized.
• Promote independence, community inclusion, and self-advocacy.
• Transport individuals to appointments and community activities.
• Maintain documentation: daily notes, progress notes, medication records, and incident reports.
• Ensure rights are safeguarded and due process followed.
