Job summary

Project ReDirect seeks a Direct Support Professional (DSP) to provide person-centered supports to individuals with intellectual and developmental disabilities in residential and community settings. The DSP will promote independence, health, safety, and community inclusion while following program policies.

Responsibilities include assisting with ISP information gathering, implementing care plans, daily living support, and documenting progress and incidents.

Qualifications

• High School Diploma or GED required.
• Experience supporting individuals with intellectual and developmental disabilities preferred.
• Must be at least 18 years of age.
• Background checks required and DDS training may be required by program.

Responsibilities

• Assist with information gathering for ISPs, planning meetings, assessments, and service reviews.
• Implement ISP goals, BSPs, Nursing Care Plans, and other approved support strategies.
• Assist individuals with activities of daily living including personal care, meals, housekeeping, and medication support as authorized.
• Promote independence, choice, dignity, self-advocacy, and community inclusion.
• Support individuals in community activities, employment, volunteering, recreation, and social events.
• Maintain a clean, safe, and healthy living environment following safety and infection control procedures.
• Observe, document, and report changes in health, behavior, or well‑being.
• Complete required documentation including daily notes and records.
