Job summary

Choices for People is seeking a Direct Support Professional to assist individuals with disabilities in achieving employment goals and community inclusion under their PCSP. The role focuses on coaching, advocacy, and tailored supports to promote meaningful work experiences and independence.

Responsibilities include collaboration with families, employers, and interdisciplinary teams, plus documentation and transportation support as needed to ensure successful community participation.

Qualifications

• Must be at least 18 years old.
• High School Diploma or equivalent preferred.
• Experience providing person-centered support to individuals with disabilities preferred.
• Ability to document services accurately using electronic documentation systems.
• Maintain confidentiality and comply with HIPAA regulations.

Responsibilities

• Establish and maintain a safe, respectful environment that promotes individual choice and growth.
• Provide person-centered support to individuals to promote employment, independence and inclusion.
• Deliver individualized services, training, and supports per PCSP guidance and policies.
• Assist individuals with transportation, errands, and community-based activities.
• Coordinate and oversee services with VR/DMH or other referral sources.
• Communicate effectively with individuals, families, employers, and team members.
