Job summary

OneWell Health Care in Reading, PA seeks a Direct Support Professional (DSP) to provide hands-on, person-centered care for individuals with IDD in home and community settings. Qualifications

• English proficiency required (spoken and written).
• High school diploma or equivalent.
• Experience in caregiving, home health, or direct support preferred.
• Ability to use electronic documentation and EVV systems.

Responsibilities

• Assist with personal care and activities of daily living.
• Support meal preparation, mobility, and medication assistance as authorized.
• Implement behavior support strategies and monitor health and safety.
