Job summary

Heartshare is seeking a Direct Support Professional in New York to implement person-centered services and support individuals in daily activities and community inclusion. You will help maintain physical and emotional well-being and participate in planning and documentation for each person.

The role requires engaging with individuals, families, and staff, and ensuring safety and privacy while promoting community participation.

Qualifications

• High School Diploma, GED or CNA strongly preferred but not required.
• One year or more experience working with individuals with developmental disabilities preferred.
• Valid NYS driver’s license preferred but not required.

Responsibilities

• Implement individuals’ plans and services aligned with goals and outcomes.
• Engage individuals in skills development, socialization and tasks across settings.
• Transport individuals or assist with public transportation as needed.
• Complete written reports and documentation on a timely basis.
• Maintain confidentiality and privacy of staff and individuals.
