Job summary

Heartshare Human Services Of New York is seeking a part-time direct-support professional to implement individualized plans, assist with activities, and promote community inclusion in Brooklyn and surrounding areas.

The role emphasizes working with individuals with developmental disabilities, maintaining detailed documentation, and coordinating with families and staff. A high school diploma or equivalent is preferred, with some experience in the field and a NYS driver’s license preferred.

Qualifications

• High School Diploma, GED or CNA preferred but not required.
• One year or more experience working with individuals with developmental disabilities preferred.
• Valid NYS driver's license preferred but not required.

Responsibilities

• Implement individuals’ plans, consistent with their specific services, goals and outcomes.
• Support and teach individuals according to their services, goals and outcomes; engage in skills development across settings.
• Promote inclusion of individuals in their community; foster personal growth and friendships.
• Maintain communication with family, advocates, and service providers as needed.
• Transport individuals or assist with public transportation as required.
• Plan recreational activities and outings; participate in PPT meetings as requested.
• Document information in medical, behavioral, goals/outcomes, fire safety, and other mandated records; ensure accuracy.
• Complete all written reports and documentation on time.
• Provide appropriate supervision; ensure staff are trained on the LOS for each person.
• Engage in all program services such as toileting, bathing, behavioral interventions, and meals as needed.

Education

High School Diploma, GED or CNA

One year or more experience with individuals with developmental disabilities

Valid NYS driver's license (preferred)
