Job summary

Qualifications

Be at least 18 years of age with high school diploma or GED.
Must communicate effectively in English, both written and oral.
Must have reliable transportation for transporting clients.
Must obtain Class E driver’s license during new hire orientation and carry required auto insurance.

Responsibilities

Provide high quality, person-centered services per ISP/BSP.
Follow plan requirements and supervise as indicated.
Document care activities accurately and timely.
Assist with daily living, personal care, meals, and community participation.
Promote safety, confidentiality, and respect for residents.
