Qualifications

Responsibilities

• Provide direct support per individual service plans, including socialization, health maintenance, and medication administration.
• Assist with daily activities such as personal care, housekeeping, transportation, and community integration.
• Accurately document progress and activity, review records, and maintain confidentiality.
• Maintain safe, respectful relationships with individuals, families, and caregivers; follow Sevita policies and customer service standards.
