Sevita is seeking a Direct Support Professional to provide direct support in residential settings, typically serving 4–6 individuals, or up to 10 in periodic programs.

The role includes delivering service plan support, health maintenance, medication administration where assigned, and assisting with daily living activities. Qualifications include a high school diploma and at least 6 months in human services; a valid driver’s license is required.

Qualificações

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsabilidades

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
• Makes cost-effective and appropriate purchases within the budget of the individual served and according to their choice / preference.
• Maintains healthy and professional relationships with individuals, families, guardians and case managers; implements Sevita Customer Service Standards.
• Reports any instance of alleged abuse or neglect and any medical/behavioral incidents as required.
• Maintains confidentiality and respects the rights of persons served.
