Urban Pathways in New York, NY is seeking a Case Manager to deliver and document clinical and case management services for assigned clients. You will listen actively and promote equitable, anti-racist practices within a supportive team environment.

The role involves field visits, collaboration on housing and services, and ensuring timely documentation. A bachelor’s degree in social work or related field is required, with at least one year of relevant experience preferred.

Qualifications

• Bachelor’s degree required; Social Work or related field preferred.
• Minimum one year of case management or counseling experience preferred.
• Knowledge of NYC housing subsidy programs helpful.
• Experience with homeless, mental illness and/or substance abuse populations preferred.

Responsibilities

• Provide and document clinical and case management services for assigned clients.
• Promote equitable, anti-racist organizational culture and customer service.
• Conduct field work including home visits and outreach.
• Maintain accurate intake, progress notes, and client records within required timelines.
• Coordinate referrals to housing, entitlements, education, employment and health services.
