Job summary

Experience Senior Living LLC is seeking a Care Partner/QMAPS to join our team in Lone Tree, CO. This PRN position involves assisting residents with personal care, daily tasks, and participating in life enrichment activities.

You will maintain infection control practices, document reports, and communicate changes in residents' needs to the health team. Qualifications include a High School Diploma or equivalent.

Qualifications

• High School Diploma or equivalent.
• Certified Nursing Assistant/Home Health Aide preferred, but not required.
• License/Certificate for Medication Assistance (if applicable) preferred per state regulations.
• CPR certification/First Aid certification preferred.
• Must complete all state and Experience Senior Living specified training programs.

Responsibilities

• Assist residents with personal care and activities of daily living as assigned and per service plan.
• Perform daily housekeeping tasks and laundry as assigned.
• Review and participate in 24 Hour Reports, shift change reports and verbal reports.
• Assist residents to and from life enrichment programs.
• Promote independence and respect residents' rights and preferences.
• Communicate changes in residents' needs to the Director of Health & Wellness and other departments.
• Respond to resident pendant/wearable calls promptly.
• Maintain infection control practices and report accidents/incidents.
