Job summary

Paycom - ATS is seeking a Physical Therapy Assistant in the home health setting in Jacksonville, FL. The role involves delivering PT services in patients' homes under a PT's supervision and coordinating with a multidisciplinary team to address mobility and functional goals.

The ideal candidate holds a Florida PT or PTA license, has reliable transportation, and maintains current CPR certification. Eligibility to work with diverse patient ages is required.

Qualifications

• Current Florida PT/PTA license from an accredited program.
• Reliable transportation to and from patient homes.
• Current CPR certification.

Responsibilities

• Participate in in-office or telephonic case conferences with the interdisciplinary team.
• Coordinate assessment findings and discharge planning with the HH treatment team.
• Instruct patients and families in care and use of physical therapy devices and plan of care.
• Provide home-based physical therapy services as prescribed by a physician.
• Evaluate patients using diagnostic tests and functional assessments.
• Supervise and mentor PTA as outlined in policy while collaborating with teachers, physicians, psychologists, social workers and other professionals.
