Job summary

AHRC New York City is seeking two Case Managers to oversee services for participants in the DYCD Train & Earn program in Manhattan. You will coordinate employment support, training, and postsecondary pathways to help youth and young adults achieve stable, meaningful employment.

You will conduct assessments, develop service plans, and collaborate with families, employers, and care coordinators while navigating city systems to maximize outcomes.

Qualifications

• Associates or Bachelor's in social work or related field.
• At least three years of case management experience with city systems.
• Strong verbal and written communication skills.
• Ability to build relationships with families, employers, and stakeholders.
• Ability to organize work and work independently.
• Experience mentoring program participants is desirable.

Responsibilities

• Coordinate support services for individuals on caseload focused on employment success.
• Assist participants with program paperwork and administrative tasks.
• Develop service plans to support employment goals.
• Address social factors in planning employment plans and financial planning.
• Conduct assessments, service plans, and progress notes.
• Provide in-person or remote supportive services as needed.
• Attend case conferences and team meetings.
• Meet with individuals and their network of supports.
• Track progress and outcomes of participants.
• Promote engagement and document training attendance.
• Guide participants and caregivers toward industry certifications.
• Deliver work readiness and occupational training curriculum.
• Maintain relationships with EBS staff for job seeker prep.
• Cover program duties during staff absences.
