Job summary

The Christ Hospital Home Health is seeking an experienced Registered Nurse to provide skilled home health care within a defined Ohio territory. The RN educates patients and families, ensures safety, and maintains accurate documentation under physician orders.

Responsibilities include conducting assessments, administering medications, coordinating with a multidisciplinary team, and supporting patient care transitions. A valid RN license and ongoing compliance with state rules are required.

Qualifications

• Minimum of one year of RN experience in an acute care setting; home health experience preferred.
• Strong communication with patients, families, physicians and coworkers.
• Calm, patient, and accommodating demeanor; compassionate patient care.
• Knowledgeable about nursing best practices and clinical judgment.
• Ability to make appropriate nursing judgments and prioritize tasks.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled nursing care at patients’ homes as ordered by the physician.
• Observe and monitor patient conditions; administer medications as prescribed.
• Perform OASIS assessments to develop and update care plans.
• Educate patients and families; ensure documentation and regulatory compliance.
• Coordinate with LPNs and aides; ensure safe transitions and care continuity.
• Document all care activities and clinical assessments in a timely manner.
