Job Summary

Registered Nurse (RN) at Christ Hospital Home Health Care. Provide comprehensive skilled care in a defined geographical territory, working autonomously and within scope of state practice. Collaborate with interdisciplinary team.

Key Responsibilities

• Observe and monitor patient conditions; perform OASIS assessments; develop and adjust individualized care plans.
• Administer medication as prescribed; prioritize visits for high-risk patients to reduce re-hospitalizations.
• Manage multidisciplinary care and promote continuity of care with admissions, transfers, and discharges.
• Educate patients and families on disease processes using teach-back methods.
• Complete all clinical documentation per agency protocol and Medicare/Federal guidelines.
• Collaborate with interdisciplinary team and physicians to ensure optimal care.
• Follow agency policies, procedures, and regulations; communicate schedule/availability changes.
• Perform other duties as assigned.

Qualifications

• Registered Nurse with current state license in the state of employment.
• Minimum one year of RN experience in an acute care setting; home-care experience preferred.
• Valid driver’s license and auto insurance in applicant’s name.
• Capable of performing all physical demands of the role.
