• Performs a comprehensive assessment on all assigned patients as evidenced by completed documentation.
• Communicates changes in patient’s condition promptly.
• Utilizes nursing diagnosis and plan of care to set measurable goals and modify care plan as needed.
• Identifies and prioritizes patient needs and documents.
• Collaborates with patient, family, and health care team in planning care and discharge planning.
• Evaluates patient responses to nursing interventions and adjusts care accordingly.
• Ensures discharge instructions and medication reconciliation are completed and shared with patient/family.
