Responsibilities

• Collects and analyzes data to develop an individualized plan and evaluates progress.
• Directs care and coordinates involvement of any assistants.
• Collaborates with patients, families, physicians and staff to document outcomes.
• Participates in governance, committees, and unit projects.
• Document patient responses and adjust care plans based on findings.
• Utilize best available evidence to guide practice decisions.
