Job summary

Responsibilities include coordinating with outreach teams, assisting with transportation, developing individualized wellness plans, and maintaining care continuity while protecting client privacy under HIPAA

Qualifications

• Lived experience with mental health or homelessness is valued.
• Ability to work with people experiencing homelessness in various settings.
• Knowledge of 12-Step programs or recovery experience is desirable.
• Understanding of public benefit programs (food stamps, TANF, SSI/SSDI).
• High school diploma or GED required.

Responsibilities

• Support outreach, engagement and linkage with homeless outreach teams.
• Provide behavioral health services and assist with transportation to services.
• Link clients to resources, develop wellness plans and assist with independent living skills.
• Meet clients in the community or at home, advocate for resources and maintain PHI privacy.
• Complete forms and reports timely; act as mandated reporter when needed.
