Job summary

Qualifications

Responsibilities

• Develop and evaluate treatment plans; provide education to patients and families.
• Create and supervise care plans including crisis intervention and bereavement support.
• Coordinate with healthcare team; involve patient and family in plans.
• Liaise with community resources; ensure smooth transitions and placements.
• Partner with the care team to deploy interdisciplinary services and achieve outcomes.
• Participate in performance improvement activities and data collection.
• Prepare professional reports, letters, and case summaries.
