Job summary

Qualifications

• Master's degree in Social Work is required.
• TX licensure via LCSW/LMSW/LMSW-AP must be held or eligible.
• Minimum 1 year of experience as stated in qualifications.

Responsibilities

• Conducts and documents psycho-social assessments to interpret patient needs, environment, and medical factors.
• Develops, implements, and evaluates treatment plans; provides education and serves as a resource to patients and families.
• Prepares and writes professional reports, letters, and case summaries; maintains thorough documentation.
• Liaisons with community resources and staff to ensure smooth transitions and placements within the service line.
• Collaborates with the healthcare team to deliver interdisciplinary care and achieve positive patient outcomes.
• Participates in performance improvement activities including data collection and continuing education.
