Job summary

Baylor Scott & White Health in Temple, TX seeks a PRN Social Worker MSW 1 to educate and assist patients and families with coping strategies. Responsibilities include coordinating crisis intervention, bereavement support, referrals, discharge planning, and psychosocial assessments alongside the healthcare team.

Under Texas State Board guidelines, you will collaborate on performance improvement initiatives, prepare professional reports, and help with care planning and community liaison tasks.

Qualifications

• Master's degree in Social Work required.
• Major in Social Work.
• 1 year of experience required.
• State TX licensure: LCSW, LMSW, or LMSW-AP required.

Responsibilities

• Conducts and documents psycho-social assessments to interpret social, environmental, and medical needs.
• Develops, implements, and evaluates treatment plans; provides education and acts as a resource.
• Provides or supervises care plans, including crisis intervention, prevention, education, advocacy, bereavement, therapy, and counseling.
• Liaises with community resources and staff; facilitates transition and placement within the service line.
• Collaborates with healthcare team to facilitate interdisciplinary care and positive outcomes.
• Participates in performance improvement initiatives, including data collection and education.
• Prepares professional reports, letters, and case summaries; interprets patient needs.
