Job summary

Baylor Scott & White Health in Rowlett, TX seeks a Social Worker MSW 1 to assist patients and families with emotional and psychosocial needs in the Emergency Department. You will coordinate crisis intervention, discharge planning, referrals, and bereavement support as part of the case management team.

Requirements include a Master of Social Work with TX licensure (LCSW/LMSW/LMSW-AP) and at least 1 year of experience. This full-time position includes benefits and PTO from Day 1.

Qualifications

• Masters' degree in Social Work (MSW).
• Licensure as LCSW/LMSW/LMSW-AP in TX preferred or required.
• Minimum 1 year of related social work experience.

Responsibilities

• Perform detailed psycho-social assessments and plan patient care.
• Coordinate crisis intervention, bereavement support, referrals, discharge planning and education.
• Collaborate with healthcare team to develop and implement care plans.
• Document assessments and maintain professional reports.
