Job summary

Qualifications

• Master of Social Work degree required.
• Licensure: TX LCSW/LMSW or LMSW-AP required.
• 1 year of Social Work experience preferred but not required.

Responsibilities

• Conduct detailed psycho-social assessments to identify patient needs and environment.
• Develop, implement, and evaluate treatment plans with patient and family involvement.
• Provide or supervise care plans including counseling, advocacy, and discharge planning.
• Coordinate with community resources to ensure seamless transitions and services.
• Collaborate with the healthcare team to support interdisciplinary care and positive outcomes.
• Participate in performance improvement activities, including data collection and reporting.
