Qualifikationen

• California RN licensure required.
• Two (2) years RN experience in acute care or case management.

Aufgaben

• Demonstrates compliance with Code of Conduct and resolves questions or concerns.
• Performs pre-admission review and inpatient admission review to ensure payment authorization.
• Acts as resource to ED staff on admission appropriateness and levels of care.
• Gathers information to facilitate discharge from ED to appropriate level of care.
• Coordinates patient transfer and discharge planning to optimize outcomes and minimize readmission.
