Responsibilities

• Act as a resource to ED staff and physicians regarding admission appropriateness and levels of care.
• Facilitate discharge planning with referrals to appropriate care settings.
• Assist in triaging transfers from other facilities and determining transferor admission needs.
• Assess ED patients and identify options other than acute hospital admission when appropriate.
• Communicate plan of care and changes to patient/family and care team.
