Montage Health is seeking an ED Clinical Care Coordinator to optimize patient flow in the Emergency Department and support proactive bed planning. You will ensure appropriate admission status and partner with care navigation and ED teams to coordinate disposition.

Minimum five years of acute care or ED experience, a California RN license, and a nursing degree are preferred. This per diem role emphasizes timely, safe transitions and effective collaboration with physicians and staff.

Formación

• Bachelor’s degree in nursing preferred.
• CA RN license required to practice in California.
• Experience in acute care or Emergency Department settings preferred.

Responsabilidades

• Serve as the ED care coordination hub gatekeeper for admissions and bed planning.
• Collaborate with ED physicians, nurses, and staff to anticipate admissions and discharges.
• Monitor patient flow, prevent delays, and escalate barriers as needed.
• Coordinate with care navigation, bed management, and ancillary services for disposition.
• Advise on patient status, care levels, and appropriate utilization of resources.
• Participate in discharge planning and transition of care with interdisciplinary teams.
• Identify barriers to admission and coordinate with transfer centers as required.
