Job summary

Eisenhower Medical Center seeks a Registered Nurse Case Manager to facilitate patient hospitalization from ED admission to discharge, coordinating with physicians, nurses, social workers and care teams to ensure timely, cost-effective care.

Requires BSN or MSN, California RN licensure, and 2 years of RN experience in acute care or case management. Certification in Case Management preferred; sign-on bonus eligible.

Qualifications

• Education: BSN or MSN required.
• California RN licensure required; Case Management certification preferred.

Responsibilities

• Demonstrates compliance with Code of Conduct and policies.
• Performs pre-admission and inpatient utilization reviews.
• Collaborates with ED staff and physicians on admission decisions and care planning.
• Coordinates patient transfers and discharge planning to ensure safe transitions.
• Uses ED tracking and medical records to identify patients needing CM intervention.
• Documents assessments and interventions and maintains discharge planning data.
