Job summary

Qualifications

• Must hold a current license from the State of Ohio.
• High School or GED (Required).
• BLS - Basic Life Support - American Heart Association.
• LPN - Licensed Practical Nurse - National Council of State Boards of Nursing.
• Ohio Board of Nursing approved medication course.

Responsibilities

• Perform initial and ongoing health status assessments.
• Document care plans and implement nursing interventions.
• Collaborate with healthcare team to coordinate patient care.
• Provide direct patient care and supervision of nursing tasks as indicated.
• Participate in process improvement and safety initiatives.
• Assist with patient and family education and discharge planning.
