CHRISTUS Health is seeking an Emergency Department Care Manager responsible for coordinating patient throughput and ensuring appropriate levels of care across inpatient and outpatient settings. The role collaborates with patients, families, multidisciplinary teams, physicians, and community partners to optimize outcomes and discharge planning with referrals to all levels of care.

Competenze

• Must have RN or LMSW in state of employment
• 3+ years of relevant clinical case/care management experience in acute care
• Familiarity with evidence-based medical necessity criteria sets
• Competency in prior authorization processes and software

Mansioni

• Assess and review admissions using criteria to ensure appropriate level of care
• Educate ED physicians and nurses about medical necessity and admission criteria
• Coordinate and facilitate patient progression across the care continuum
• Plan discharges with referrals to Home Health, DME, placement, and community resources
• Initiate prior authorization for post-ED referrals and services
• Manage high-use patients and seek alternatives to frequent ED visits
• Collaborate with interdisciplinary team to ensure timely care and outcomes
