Job summary

Montage Health is seeking an ED Clinical Care Coordinator to act as the gatekeeper of the Emergency Department, providing early visibility of admissions for proactive bed planning and appropriate statusing.

You will collaborate with the Care Navigation Hub, ED physicians, nurses, and interdisciplinary teams to optimize patient flow, coordinate dispositions, and ensure timely transitions to inpatient, observation, or outpatient pathways.

Qualifications

• Bachelor’s degree in nursing preferred.
• California RN license required.
• Minimum five years of acute care hospital experience preferred.
• Experience in care coordination, utilization management, and interdisciplinary collaboration preferred.

Responsibilities

• Serve as the ED clinical spoke of the Care Navigation Hub, monitoring ED demand, boarding volume, admissions, and barriers to patient progression.
• Advise bed placement to accommodate capacity constraints and prevent boarders.
• Collaborate with ED physicians, charge nurses, and interdisciplinary staff to identify admissions, observation, discharges, and care needs.
• Track patient flow and throughput, escalating barriers to avoid delays.
• Coordinate with the Care Navigation Hub to facilitate appropriate patient placement and disposition.
