Job summary

Dilkon Medical Center is seeking an ED Clinical Care Coordinator to lead clinical care coordination in the Emergency Department. You will work with the multidisciplinary team to ensure timely, quality care and effective patient flow.

Requires RN licensure and ED experience; bilingual Navajo/English is preferred. The role emphasizes advocacy, coordination with admissions, and adherence to confidentiality and regulatory standards.

Qualifications

• Completion of an accredited nursing program and an unrestricted RN license.
• Two years of clinical and/or ED nursing experience or two years in a supervisory/management role.
• Case management experience and/or certification preferred.
• BLS, ACLS, and PALS required for ED employment.
• Bilingual English and Navajo preferred.

Responsibilities

• Exemplifies professional nursing practice with leadership and management activities.
• Coordinates patient care activities and flow between ED and other units.
• Acts as liaison between ED staff, patients, and departments to ensure safe care.
• Collaborates with admitting for timely placement of admissions and transfers.
• Performs post-ED discharge calls for high-risk patients and follows up with PCPs.
• Maintains confidentiality and adheres to policies and regulations.
