Job summary

Saint Francis Hospital and Medical Center in Hartford, CT seeks a Nurse Senior – Navigator (Complex Care Coordinator) to support emergent and high-risk patients in the ED. The role partners with the ED team to deliver coordinated, patient-centered care and safe transitions across the continuum.

The RN collaborates on individualized care planning, coordinates inpatient referrals, and utilizes community resources to promote optimal dispositions.

Qualifications

• Graduation from an accredited nursing program, with a BSN strongly preferred for successful candidates.
• Valid RN license in Connecticut.
• Minimum of three (3) years of Emergency Department and/or acute inpatient nursing experience, with demonstrated ability to manage complex, high-risk patients.

Responsibilities

• Conduct comprehensive, real-time assessments of patients’ medical, psychosocial, and behavioral health needs upon presentation to the Emergency Department.
• Collaborate closely with ED physicians, APPs, nursing staff, case management, and ancillary teams to support individualized care planning for complex and high-risk patients.
• Coordinate referrals and connections to inpatient services, specialty providers, community resources, and support programs to promote safe dispositions and effective transitions of care.
