Saint Francis Healthcare System is seeking an Emergency Department Care Coordinator, a Registered Nurse responsible for managing patient care outcomes and ensuring quality service delivery in a collaborative environment. This role requires strong clinical expertise and a commitment to patient-centered care.
The Emergency Department Care Coordinator is a Registered Professional Nurse who demonstrates expanded knowledge and experience in providing care for an identified clinical specialty or case type. Under the supervision of the Director of Case Management, the Care Coordinator is responsible and accountable for desired clinical and cost-effective outcomes, collaborative practice, and the allocation of human and material resources needed for care delivery and discharge planning. The Care Coordinator uses the case management process to improve the quality and efficacy of care provided. An integrated and coordinated approach to patient outcomes is provided through the Care Coordinator, who monitors care delivery and interdisciplinary clinical practice patterns to achieve a balance between quality and cost (desired outcomes). The Care Coordinator practices according to established standards for case management and has attained competency. The Care Coordinator demonstrates the knowledge and skills necessary to provide case management appropriate to the patient's age. The Care Coordinator assesses data reflective of the patient's status and interprets the appropriate information to provide quality care. They perform utilization review duties in the Emergency Department, including guiding ED and on-call physicians regarding admission/observation criteria and identifying emergency medical conditions outlined by EMTALA to mitigate potential violations.
