Qualifications

• Master’s degree in Social Work, Psychology, Counselling, or related field.
• Experience working with substance abuse and mental health populations.
• Licensed Professional Counsellor (LPC), Licensed Social Worker (LSW) within twelve months of hire.

Responsibilities

• Assess patients for acute mental health crisis and substance concerns in ED and medical/surgical units.
• Develop treatment plans and provide direct patient services.
• Coordinate care with treatment team, patient, and family; document in medical records.
• Discharge planning and continuation of care.
• Document in medical records according to standards and regulatory guidelines.
• Respond to shifts in work assignments and may be on-call as required.
