LCMC Health in Louisiana seeks a Care Manager who serves as the hub of patient case management and care planning, ensuring quality through the continuum of care and coordinating with the care team. You will develop individualized care plans, make outbound calls to assess health status, identify barriers, coordinate care and referrals, and ensure proper discharge planning with medication reconciliation.

If you bring nursing expertise, a Louisiana license, and at least two years in care management

Qualifications

  • Current nursing license to practice in Louisiana as defined by the Louisiana State Board of Nursing.
  • 2 years of professional nursing experience in Care Management or ED.

Responsabilités

  • Develop individualized care plans with prioritized goals and involve patients and caregivers.
  • Make outbound calls to assess patient health status.
  • Identify gaps or barriers in treatment plans.
  • Coordinate care for members and arrange referrals to outside sources.
  • Ensure discharge planning includes necessary services and medication reconciliation.
  • Triage symptom-based calls per approved protocols.
  • Enroll patients and manage alerts for home monitoring programs.
  • Educate enrolled members on chronic and acute disease processes.
  • Document workflows in EPIC for tracking and quality assessment.
