Job summary

Socket.dev is seeking a Behavioral Health Case Manager to support patients in the Emergency Department and hospital settings. You will collect histories, assess functioning, coordinate admissions, and work closely with providers, payers, and crisis services.

You will arrange bed searches, transfers, and transportation while updating families. This role requires CPT/PCI training and BLS certification, with emphasis on reducing unnecessary hospitalizations.

Qualifications

• Bachelors degree in human services or related field is required.
• 13 years experience with mental health, substance use, or co-occurring disorders preferred.
• Experience using electronic health record documentation systems.
• Proficiency with Microsoft Office and internet search is expected.
• Crisis Prevention Intervention Training within 1 month of hire.
• Current BLS certification.

Responsibilities

• Responds promptly to minimize time in the ED and reduce overcrowding.
• Takes behavioral health history including past admissions.
• Collects information on functioning at home, school, community, and related factors.
• Identifies current providers and state agency involvement.
• Identifies health-related social needs (HRSN) such as housing or insurance needs.
• Provides updates to patient and guardians as discharge progresses.
• Presents assessment information to the payer and obtains authorization for services.
