About the Job:
Under the direct supervision of the Manager of Case Management, the RN Supervisor Nurse Case Management provides support to RN case managers for effective care coordination, discharge planning and transitional care as well as maintaining a caseload to assist the department as needed. Responsibilities also include, but are not limited to, the active facilitation of post-acute care planning, the provision of direct clinical supervision to the nursing staff with a focus on the effective functioning of discharge planning and transitional care, orientation of new staff, appropriate documentation, education of established workflows, and the referral management system. The supervisor will support and educate staff surrounding strategic initiatives, best practice and complex case management. Supervisor will assist in coordinating weekly schedule for discharge planning. This person is also responsible for reviewing long length of stay and/or complex cases to decrease barriers to discharge.

Bachelor's degree in a healthcare-related field or equivalent experience Current BSN or commitment to achieving the degree in a specified amount of time 3 – 5 years of case management-related health care experience. Supervisory or managerial experience required. Demonstrated knowledge of the case management and discharge planning process also required. New Hampshire Registered Nurse or Compact State Registered Nurse License CCM or ACM preferred. Excellent clinical documentation, and successful usage of EMR systems preferred Ability to advance basic computer skills as the health system adds new systems or performs upgrades to existing systems.
